We pride ourselves on our ability to accept most major medical insurance plans.   It is our goal to be able to provide ENT medical and surgical services to the most patients in our area.<\/p>\n\n\n\n

We submit claims for you only to participating insurance plans. If you subscribe to a plan that we do not participate with, then you are responsible for handling the claim. We will provide you with an itemized receipt including insurance codes at the end of your visit. CO-PAYMENTS AND DEDUCTIBLES are due at the time of your visit.<\/p>\n\n\n\n

Please present your insurance card at each visit. Appointments will be rescheduled if proper insurance identification is not made available.<\/p>\n\n\n\n

SPECIAL NOTE TO OUR HMO PATIENTS<\/h2>\n\n\n\n

If you are a member of an HMO, please remember that it is your responsibility to keep track of and arrange for referrals for your visit to our practice. Your HMO will not cover charges for your visit unless a proper referral is received in our office by the time the physician sees you. If a referral from your primary care physician is not received in a timely fashion, then your appointment will be delayed or need to be rescheduled for a later date.<\/p>\n\n\n\n
